  1. Open the Membership Application, complete the requested information, and print your completed form.
  2. Open the Member Identity Verification Form, complete the requested information, and print your completed form.
    To Join:
    • in Person: Do not sign this form until you bring it to your credit union branch office (we will verify your signature).
    • via Mail: You must sign this form in front of a Notary Public.
  3. Make photocopy of your “Hospital ID Badge” and Driver’s License (or official Government Picture ID)
  4. Bring (or mail) the above documents to the branch office that will be servicing your account.
  5. The full credit union name is Health Employees Federal Credit Union.
